Poutini Waiora is a Kaupapa Māori Health and Social Service Provider working across Te Tai o Poutini (Kawatiri, Māwhera, Hokitika/Westland, South Westland). providing a range of free and accessible health and social services.

Mō te tūnga | About the role

We are contracted by Te Tauraki, the Whānau Ora Commissioning Agency for Te Waipounamu, to deliver whānau ora services through the employment of a Kaitūwhana. 

 The Kaitūwhana works in partnership with whānau who meet the referral criteria, supporting them to identify their needs and aspirations, navigate complex situations, and advocate for access to appropriate services and opportunities. 

 The Kaitūwhana acts as a connector, advocate, and coach, guiding whānau to navigate pathways in health, housing, education, employment, and wellbeing, while reinforcing their tino rangatiratanga and whakapapa links to mana whenua, hapū, and whenua. This role is pivotal in connecting whānau with their communities, and to awhi and tautoko them to improve their overall circumstances.

  • Assisting whānau/whaiora to develop plans for their current and future needs

  • Risk management

  • Crisis response

  • Facilitating access to resources

  • Providing tailored support and guidance

  • Upholding cultural values to promote holistic wellbeing

To ensure they are well equipped to address the complex challenges faced by whānau, the Kaitūwhana will participate in ongoing supervision and professional development.

Desirable:
  • Fluency in Te Reo Māori
